SOLLIS HEALTH MEMBERSHIP CODE OF CONDUCT
We have developed this Code of Conduct to ensure a safe, friendly and respectful environment for all members, guests, visitors and staff. We value all of our members and want to provide them with the highest-quality care and services while fostering a comfortable and effective provider-member relationship.
- Respect: Sollis Health is committed to providing a safe and harmonious environment for our members, guests and staff. Everyone has the right to be treated fairly and with dignity. Members are expected to be respectful to and civil with all other Sollis Health members and employees. Members should not knowingly denigrate, insult, berate, abuse or raise their voice at staff members, other members and/or guests, nor should they create unnecessary strife and divisiveness through any manner.
- Privacy: Sollis Health members are responsible for respecting the privacy of other members. We ask they not approach other members unless already personally acquainted.
- Confidentiality: Sollis Health members should maintain strict confidentiality standards concerning other member’s personal information. A member shall not disclose any information, medical or otherwise, about other members to nonmembers, outside agencies or commercial enterprises in any form, e.g. written, electronic, social media or verbal.
- Harassment-Free Environment: Sollis Health maintains an environment free from all forms of harassment and discrimination as defined by federal, state and local laws, e.g. Title VII, Americans with Disabilities Act, Genetic Information Nondiscrimination Act.
- Obscenities: Sollis Health locations are environments for all ages. They should remain free of obscenities and inappropriate behavior. All communication and action should be respectful to staff, physicians and other members.
- Drugs/Alcohol: Sollis Health locations and sponsored events will remain drug and alcohol free unless under medical supervision. with the exception of medical interventions as deemed appropriate and necessary by licensed Sollis Health medical providers.
- Non-Solicitation: Sollis Health members are to refrain from solicitation including but not limited to business or fundraising efforts and promotions.
- Medical Plan of Care: All members receiving care through Sollis Health will be assessed and treated in accordance with our provider's orders. The plan of care will be individualized according to the needs of the member. Members will follow the care plan discussed and decided by you and your medical care team in accordance with Sollis Health processes.
Breaches of this Code of Conduct:
Any Sollis Health member who does not behave in accordance with the terms of the Code of Conduct and/or who repeatedly disrupts the activities of Sollis Health will be subject to investigation and disciplinary action. Inappropriate behavior of any members, visitors or staff will be investigated. Appropriate course of action will be taken by Sollis Health leadership including but not limited to: verbal or written warning, temporary membership suspension or the immediate revocation of membership. A member in violation may be asked to leave the location and be subject to termination of membership without entitlement of a monetary refund.
Reporting Violations:
If a Sollis Health member, guest or staff feels that a member, guest or staff of the Sollis Health community is violating the Code of Conduct guidelines in their interactions, please reach out to memberservices@sollishealth.com
