How Sollis Accesses Your Medical History for Better Care

If you're like most people, your medical records don't live in one place. So what happens when you have an emergency?

When you walk through the door at Sollis (or reach us by phone in the middle of the night), our clinical team wants to understand your health as fully as possible, as quickly as possible. But if you're like most people, your medical history doesn't live in one place. You may have a primary care doctor in one part of the city, a specialist across town, and a history of urgent care visits from years ago scattered across different health systems in other cities and states.

The unfortunate truth is that most of us lose track of our medical records. We move and change insurance providers. Doctors and portals don't always talk to each other. And in an urgent moment, there simply isn't time to track down paperwork.

This is where the health information exchange (HIE) comes in. With your consent, Sollis participates in an established HIE network that gives our providers secure near-instant access to relevant medical records from the majority of outside healthcare organizations.

Here's how it works—and why it matters for your care.

How Sollis Uses the HIE to Support Your Care

Urgent and acute care decisions happen fast. Having timely access to your relevant medical history doesn't just improve efficiency—it can genuinely change outcomes.

Consider a few common scenarios where we use the HIE at Sollis:

  • You come in with chest pain. Before our provider orders a full workup, they can quickly review whether you had a cardiac evaluation recently, what it showed, and whether your current symptoms represent a new development or a known pattern.
  • You're seen for a possible infection. Rather than guessing at your antibiotic history, our provider can pull up recent prescriptions and lab cultures to make a more informed treatment decision from the start.
  • You need imaging. If a recent scan already exists in the HIE network, our team can review it, avoiding redundant testing and unnecessary cost or radiation exposure.

This is the practical value of HIE access: more informed clinical decision-making, better continuity across care settings, and a more complete picture of your health in the moments it matters most.

It's worth noting that HIE access complements, not replaces, the conversation your provider has with you. Your account of your symptoms, your history, and your concerns always remain central to your care.

What Is a Health Information Exchange (HIE)?

A health information exchange is a secure electronic network that allows healthcare organizations to share patient health information with one another. Think of it as a trusted encrypted bridge connecting hospitals, labs, imaging centers, specialist offices, and primary care providers across different health systems.

Through the HIE, authorized clinicians can access records like:

  • Lab results and blood work

  • Imaging reports (X-rays, MRIs, CT scans)

  • Hospital discharge summaries

  • Medication histories and allergy records

Sollis participates in an established HIE so our providers aren't starting from scratch every time you come in for care. Your information is accessed only with your consent, and only when it's clinically relevant to what you're being seen for.

It's important to know that while the majority of our outside providers participate in the HIE network, not all do. For any clinically important records that aren't available electronically, our team can arrange for a manual transfer.

How Sollis and Your PCP Work Together

Sollis is not a replacement for a primary care provider (PCP). We work with your primary care provider and other doctors to close the gap between primary and urgent and emergency care. Using the HIE allows us to connect with your PCP more seamlessly, providing you with faster, better care without sacrificing privacy or security: your PCP stores and manages your long-term health story, while Sollis uses HIE access (with your consent) to understand recent or relevant care at the moments when it matters most.

  • Your PCP is the central hub of your long-term health. They maintain a comprehensive view of your medical history: your preventive screenings, chronic condition management, medication changes over time, and the kind of longitudinal trends that only emerge across years of care. That relationship and those records are irreplaceable.
  • Sollis is designed for episodic, urgent, and acute care needs. When something comes up that can't wait—like an injury, a sudden illness, or a symptom that needs same-day evaluation—Sollis is built to respond immediately. The HIE allows our providers to access relevant outside records in that moment, so that your recent history informs the care you receive right now.

Don't have a PCP? Primary care referrals are included in your Sollis membership. If you don't currently have a PCP, or you'd like help connecting with one, our team is happy to assist with a referral. We can even expedite the first appointment for you.

At Sollis, you're always in control of your information. We access outside records through the HIE only with your explicit consent—and giving that consent doesn't require you to gather or send records yourself. A simple authorization form is all it takes. If you ever change your mind, your consent can be withdrawn at any time, for any reason.

On the security side, Sollis follows HIPAA and industry-standard data security practices. Only authorized clinical staff can access your information through the HIE, and only in the context of providing your care. Your records are not used for marketing, shared unnecessarily, or accessed outside of clinical need.

Why HIE Reduces the Need for Manual Records Requests

Anyone who has tried to transfer medical records between providers knows how frustrating the traditional process can be. You might spend days:

  • Waiting for a fax to go through (and confirming it was received)
  • Logging into multiple patient portals to download individual documents
  • Calling outside offices repeatedly to follow up on pending transfers

These delays aren't just inconvenient—they can meaningfully affect care, especially when a provider is trying to make an informed decision quickly.

The HIE largely eliminates this friction. When your consent is already on file and your care team has network access, relevant records can often be reviewed in minutes rather than days.

Your Care, Informed and Secure

At Sollis, access to your medical history isn't just about convenience. It gives our providers the information they need to take the best possible care of you when something urgent comes up. The HIE makes that possible without burdening you with the logistics of manual record gathering.

Here's how to get started:

  • Provide or confirm your HIE consent so our team can access relevant records when needed
  • Let us know where you've received care recently by sharing any hospitals, specialists, or health systems we should be aware of
  • Reach out with questions about how your records are shared or what information we may access
  • Ask us about primary care coordination if you'd like help connecting with a PCP
