Wildfire smoke contains tiny particles called particulate matter. The most harmful particulate matter is called PM2.5. These particles are 2.5 microns or smaller, about 30 times thinner than a human hair. Because they are so small, they can reach deep into your lungs. Some particles can even enter your bloodstream.2

Research suggests wildfire smoke may harm your lungs more than other types of air pollution. A study found that hospital visits for breathing problems rose by as much as 10% when wildfire PM2.5 increased by 10 μg/m³. Other pollution sources of PM2.5 caused only 0.67% to 1.3% of respiratory-related hospital visits.3

Wildfire smoke differs from regular city air pollution. Burning trees, homes, and other items creates a mix of gases and particles. These can cause health effects right away or days later. The CDC notes that smoke can travel hundreds of miles. You can be affected even if you live far from the fire.

Both your lungs and heart respond to smoke. The tiny particles cause swelling in your airways. This leads to coughing, wheezing, and trouble breathing. Some studies show smoke can also affect your blood vessels and heart rhythm.9

Most people experience relief from acute symptoms like cough, irritation, and shortness of breath within a few days after smoke clears. However, emerging research shows underlying cardiorespiratory effects may persist for weeks to months, particularly in individuals with pre-existing lung or heart conditions. Some people exposed to intensive, prolonged smoke may experience measurable changes in lung function for one to two years.10

Delayed symptom onset is not uncommon. Some people feel effects right away, while others notice symptoms 24 to 72 hours later. This means you may be exposed while still feeling fine. Monitor air quality daily throughout the smoke event and for several days after. Recognizing Wildfire Smoke Symptoms in Southern California

Wildfire smoke causes many symptoms that range from mild to severe. The CDC lists common signs: headache, eye irritation, trouble breathing, cough, wheezing, chest pain, fast heartbeat, and tiredness.1

Breathing symptoms often show up first. You may have a dry cough that will not go away, wheezing, or chest tightness. Shortness of breath during normal activities means smoke is affecting your airways. Throat irritation is common even with light smoke exposure.

Heart-related symptoms need close attention. The Environmental Protection Agency notes that people with heart disease who breathe in fine particle pollution, which includes wildfire smoke, could experience chest pain or tightness, a fast or irregular heartbeat (palpitations), unusual fatigue, and shortness of breath.11 

If you have a history of heart disease, diabetes, high blood pressure, or high cholesterol, inform your doctor of smoke exposure. Wildfire PM2.5 is linked to acute coronary syndrome, arrhythmia, and heart failure flare-ups. Do not delay seeking care if you experience chest pressure, palpitations, or unusual fatigue during or after smoke events.

Eye irritation, headaches, and fatigue are also common. Many people feel a burning in their eyes, more tearing, or trouble wearing contact lenses when the air is smoky.

If you start with just eye irritation but then develop chest tightness or trouble breathing, the situation has become more serious. If you have asthma, smoke can make it worse. You may cough more, wheeze more, and need your rescue inhaler more often. CDC data from 2023 showed 17% more asthma ER visits during heavy smoke days.5

Who Is Most at Risk from Wildfire Smoke in Southern California

Wildfire smoke affects everyone, but some groups face higher risks. The EPA lists several groups that are more likely to have health problems from smoke.6

Children are at high risk. Their lungs are still growing. They breathe more air for their body size than adults. Kids also spend more time playing outside, which increases their exposure. During big wildfire smoke events, more people with asthma end up in the ER, including kids, as smoky air can trigger asthma attacks.5 Young children may not know how to tell you they feel bad. Watch for signs like fast breathing, flared nostrils, or skin pulling in between their ribs when they breathe.

Adults over 65 face greater risks. Many have heart or lung conditions already. Lung function also drops as we age. This age group makes up a large share of smoke-related hospital visits.

Pregnant women need to be careful. Limited evidence suggests smoke during pregnancy may raise the risk of early birth and lower birth weight.7 One review found support for these links, especially when exposure happened late in pregnancy.8 If you are pregnant, talk to your doctor about how to protect yourself.

People with asthma, COPD, heart disease, or diabetes face much higher risks. Smoke can trigger flare-ups of these conditions. This leads to more medication use, ER visits, and hospital stays.4

Outdoor workers face extended exposure. This includes people in construction, landscaping, farming, and delivery jobs. They often cannot avoid being outside during smoky days. Cal/OSHA has rules to protect workers when air quality is poor.

How you respond to smoke varies a lot. Even healthy people may have symptoms during heavy smoke events. Your fitness level, how much time you spend outside, and how hard you are working all play a role.

When to Seek Emergency Care for Smoke Exposure in Southern California

Some symptoms need emergency care right away. Call 911 if you have:

  • Severe trouble breathing or cannot catch your breath

  • Chest pain or pressure, especially if it spreads to your arm, jaw, or back

  • Confusion, trouble staying awake, or trouble speaking

  • Blue color on your lips, face, or fingernails

See a doctor soon if you have: a cough that keeps you from sleeping, breathing trouble that does not get better with rest, or symptoms that get worse even when you stay inside.

How to Protect Yourself from Wildfire Smoke in Southern California

Several strategies can reduce your smoke exposure and protect your health during fire season:

  • Stay inside with windows and doors closed. If you have AC, set it to recirculate so it does not pull smoky air from outside. Close any fresh-air intakes on your HVAC system.

  • Use HEPA air filters. They can lower indoor smoke levels significantly. How well they work depends on your room size, filter quality, and how well your home keeps out outdoor air.6 Put a portable air cleaner in the room where you spend the most time, like your bedroom.

  • Wear N95 masks outside. They protect you when you must go outside, but only if they fit right. Regular cloth or surgical masks do not filter smoke particles. Look for NIOSH-approved N95 masks and make sure the mask seals tight to your face with no gaps.

  • Check air quality regularly. Visit AirNow.gov or the EPA Fire and Smoke Map for real-time PM2.5 readings and forecasts to help you plan your day.

  • Create a clean air room. Pick a room with few windows. Seal gaps around doors and windows. Run a HEPA air purifier all the time. The bedroom often works well since you spend many hours there sleeping.

  • Avoid adding to indoor pollution. Do not burn candles or use gas stoves without venting. Avoid vacuuming unless you have a HEPA filter. Do not smoke indoors. Keep activity light so you breathe slower and less deeply.

  • Stock up before fire season. Keep extra air filters on hand. Have a supply of N95 masks that fit you well. Make sure you have enough medicines to last two weeks.

  • Know when to keep children home. If air quality reaches Unhealthy for Sensitive Groups (AQI 101-150) and your child has asthma, respiratory symptoms, or outdoor activities are planned, consider keeping them home. At AQI 151 or higher, many schools issue closures. Check local guidance.

  • Outdoor workers: discuss modifications with your employer. If you work outdoors and AQI exceeds 100, ask about indoor tasks, earlier or later hours, or respirator provision per Cal/OSHA requirements.
